Mindd International Forum 2019 – Practitioner Training in Functional Medicine

Competition Terms and Conditions

1. The promoter is Designs for Health Australia Pty Ltd (ABN: 11 623 343 804) whose registered office is at 1/418 Pittwater Rd, North Manly, NSW, 2100. Australia.
2. The rules of the Two Full Access Practitioner Tickets competition and how to enter are as follows:
   Place an order valued at over $150 inc. GST (freight free) either via the Designs for Health website, via phone to Designs for Health or via the practitioner’s Designs for Health direct sales contact to receive one entry into the competition before COB, Friday, March 8th. For every order of $150 you place directly with Designs for Health, you will get another entry to win.
3. The Full Access Practitioner Tickets prize is as follows:
   Two (2) Practitioner Full Access Tickets to the Mindd International Forum 2019 which will be held at the Hilton, Sydney on 23rd & 24th March 2019.
   Prize includes twin room accommodation for two people (hotel to be advised)
   The prize package outlined above is valued at $2,500 (AUD)
4. The rules of the Live Stream tickets competition and how to enter are as follows:
   For Live Stream tickets to Mindd International Forum 2019 register as a Designs for Health practitioner via the Designs for Health Australia website  https://designsforhealth.com.au/ to receive one entry into the competition.
5. The Live Stream ticket prizes are as follows:
   One of TEN available Live Stream Practitioner Tickets to the Mindd International Forum 2019
   Each Live Stream ticket prize is valued at $993 (AUD)
6. Winners will be chosen at random from all entries received and verified by Promoter and or its agents by:
   • Friday the 8th of March for Two Full Access Practitioner Tickets
   • Friday the 15th of March for Live Stream tickets
7. The competition is open to residents of Australia aged 18 years or over except employees of Designs for Health Australia Pty Ltd and their close relatives and anyone otherwise connected with the organisation or judging of the competition.
8. There is no entry fee necessary to enter this competition.
9. By entering this competition, an entrant is indicating his/her agreement to be bound by these terms and conditions.
10. Marketing of this competition has been promoted on both Instagram and Facebook.
11. Only one entry will be accepted per person. Multiple entries from the same person will be disqualified.
12. Closing date for entry will be Thursday the 14th of March, 2019. After this date, no further entries to the competition will be permitted.

13. No responsibility can be accepted for entries not received for whatever reason.
14. The prizes are as stated, and no cash or other alternatives will be offered. The prizes are not transferable. Prizes are subject to availability and we reserve the right to substitute any prize with another of equivalent value without giving notice.
15. The promoter reserves the right to cancel or amend the competition and these terms and conditions without notice in the event of a catastrophe, war, civil or military disturbance, act of God or any actual or anticipated breach of any applicable law or regulation or any other event outside of the promoter’s control. Any changes to the competition will be notified to entrants as soon as possible by the promoter.
16. The promoter is not responsible for inaccurate prize details supplied to any entrant by any third party connected with this competition.
17. The winners will be notified by email or phone within 4 days of the closing dates. If the winner cannot be contacted or do not claim the prize within 5 days of notification, we reserve the right to withdraw the prize from the winner and pick a replacement winner.
18. The promoter will notify the winner how to redeem the prizes.
19. The promoter’s decision in respect of all matters to do with the competition will be final and no correspondence will be entered into.
20. By entering this competition, an entrant is indicating his/her agreement to be bound by these terms and conditions.
21. The competition and these terms and conditions will be governed by Australian law and any disputes will be subject to the exclusive jurisdiction of the courts of Australia.
22.  The winner agrees to the use of his/her name and image in any publicity material, as well as their entry. Any personal data relating to the winner or any other entrants will be used solely in accordance with current Australian data protection legislation and will not be disclosed to a third party without the entrant’s prior consent.
23. The winner’s names will be available 5 days after closing dates by emailing the following address: info@designsforhealth.com.au
24. Entry into the competition will be deemed as acceptance of these terms and conditions.
25. This promotion is in no way sponsored, endorsed or administered by, or associated with, Facebook, Twitter or any other Social Network. You are providing your information to Designs for Health Australia and not to any other party.
26. Designs for Health Australia ‘s decision as to those able to take part and selection of winners is final. No correspondence relating to the competition will be entered into.
27. Designs for Health Australia also reserves the right to cancel the competition if circumstances arise outside of its control.
