The test request form has been submitted to the Designs for Health AU team.
• You will receive a confirmation email with a copy of the completed form for your records
(Important – please check your junk mail folder!)
NEXT STEP – GI-MAP Test PAYMENT by your patient
As you selected ‘Patient to Pay‘, they will receive a confirmation email that includes the payment link.
Please tell your patient to expect the email and to pay promptly so the test kit can be sent to them directly.
NOTE: The GI-MAP test Kit cannot be sent prior to full payment being received.
