You will receive a confirmation email with a copy of the completed form for your records
(Important – please check your junk mail folder!)

NEXT STEP – OMX Test PAYMENT

As you selected ‘Practitioner to Pay’ on the DFH Webstore, you can make the payment below.

Please pay promptly so the test kit can be sent to your patient directly.

NOTE: The OMX Test Kit cannot be sent prior to full payment being received.
