The test request form has been submitted to the Designs for Health AU team.
- You will receive a confirmation email with a copy of the completed form for your records

NEXT STEP – OMX Test PAYMENT by your patient
As you selected ‘Patient to Pay‘, they will receive a confirmation email that includes the payment link.
Please tell your patient to expect the email and to pay promptly so the test kit can be sent to them directly.
NOTE: The OMX test kit cannot be sent prior to full payment being received.
