The test request form for your patient has been submitted to the Designs for Health AU team.
You will receive a confirmation email with a copy of the completed form for your records.
(Important – please check your junk mail folder!)
NEXT STEP – GI-MAP Test PAYMENT
As you selected ‘Practitioner to Pay‘ with your pre-approved account, you will receive a confirmation email advising your account will be charged.
We will arrange payment with your account so the test kit can be sent to your patient directly.
NOTE: The GI-MAP Test Kit cannot be sent before full payment being received.
